1. Fill out the AYSO Incident Report form Do this right away, then give it to the RC.
ASYO Incident Report Form

2. Fill out the Notification of injury form (with in 90 days of the injury).
This form is what you use to submit medical bills to the AYSO insurance:

For injuries PRIOR to July 1st 2007, use the SAI form English - Rev 9 -06 (Loomis)
>Loomis form (for Prior to July 1st 2007)

For injuries AFTER July 1st 2007, use the SAI claim Form English (Chubb)
Chubb form (for after July 1st 2007)
